Terms and Conditions

1. NATURE OF WELLNESS PASSThe Wellness Pass is a time-limited wellness program providing access to a defined set of wellness experiences during a fixed participation period.This pass is not a gift certificate, gift card, or stored-value product and does not represent monetary value. It grants access to scheduled wellness services only during the stated program period.
2. PROGRAM PERIOD & EXPIRATIONThe Wellness Pass is valid for 90 days from the date of purchase (“Program Period”).All wellness experiences must be scheduled and used within the Program Period. Unused experiences expire at the end of the 90-day period and do not carry forward.No extensions, pauses, or rollovers are provided, except where required by law.
3. INCLUDED BENEFITSThe Wellness Pass includes up to five (5) wellness experiences offered by participating partners, which may include:

• Cryo / recovery / wellness services (CryoBahr)
• Wellness therapy services (SunMed)
• Fitness or training sessions (HY5 FORMERLY FitLab)

Specific offerings, durations, and scheduling availability are determined solely by each participating provider.
4. SCHEDULING & AVAILABILITY

• All services are subject to provider availability 
• Advance booking is required
• Missed appointments, late cancellations, or no-shows may be considered redeemed at the provider’s discretion
• Completion of all benefits is not guaranteed and depends on participant scheduling and availability

5. NON-TRANSFERABLE; SINGLE PARTICIPANTThe Wellness Pass is issued to one individual only and may not be transferred, resold, or shared unless explicitly stated in writing.
6. NO CASH VALUE; NO REFUNDSThe Wellness Pass:

• Has no cash value
• Cannot be redeemed for money or credit
• Is non-refundable, except where expressly required by California law

7. INDEPENDENT PROVIDERS & LIABILITY Each participating business operates as an independent provider and is solely responsible for its services, staff, facilities, safety, and outcomes.The Wellness Pass organizer does not:

• Provide medical advice
• Control individual treatment decisions
• Assume liability for services rendered by partner providers  

Participants agree to release and hold harmless the Wellness Pass organizer from claims arising from services provided by partner businesses.
8. HEALTH & PARTICIPATION DISCLAIMERParticipants acknowledge that:

• Wellness and fitness services may carry inherent risks
• They are responsible for determining whether services are appropriate for their health
• They should consult a licensed medical professional before participating if needed

9. MODIFICATIONS & PROGRAM CHANGESParticipating providers and specific offerings may change due to availability or operational needs. Comparable substitutions may be offered at the organizer’s discretion.
10. GOVERNING LAWThese Terms & Conditions are governed by the laws of the State of California.
